You set up your payment through them, and there will be a message box when you make your paypal payment and you write your current ingame name and the name you want to change it to. If you can't use paypal, I would suggest you forum pm entropy and tell him what you want to do and ask him what other methods of payment he will accept (and where to send it and who to make it out to, etc.).
